This registration form is for captains only.  When filling out the roster please indicate shirts sizes for each player as follows:

"Name of player" (S, M, L, XL, or XXL)

for example: Cheryl Miller (M)

Upon receiving this registration a member of our staff will contact you with more details in reference to shirt color and scheduling details. 

The Final Team Balance is due online by the league start date or onsite the first night (or forfeiture of the first game).  Teammates can sign up individually online and indicate this team during the process.
